请教一个税表的问题！

今天身体帐号，里面的税表不像其他公司那样在web填信息的，是需要下载 DOC 文件，然后填好了发到他们公司的邮箱或者邮寄出去的，请问下面的 签字 一栏，是否真的是需要手写的签字呢？

FOREIGN CERTIFICATION

_______________________________________ , hereinafter known as “the affiliate”, represents that it does not have any employees or equipment located in the United States (U.S. Activities) that are involved in any way with the revenue that will be earned by the affiliate pursuant to its agreement with IPNIC DBA xxxxxxxxxx.com.
U.S. activities includes, but is not limited to, owning a web server or hosting service in the U.S. or having employees in the U.S. who are involved in either

(1) Setting up the affiliate’s web server/hosting service/website
(2) Developing content for the affiliate’s Site
(3) Marketing to create a user base for the affiliate’s Site
(4) Telephone support for the affiliate’s Site
(5) Buying products (if any) for the affiliate’s site or;
(6) Maintaining the affiliate’s Site.

Notwithstanding the above representation, if the U.S. taxing authorities subsequently determine that the affiliate and IPNIC DBA xxxxxxxxxx.com, the affiliate will be responsible for providing IPNIC DBA xxxxxxxxxx.com with a Valid Certificate of the affiliate’s Exemption from obligation to pay U.S. withholding taxes as authorized by the U.S. taxing authorities and the affiliate will be responsible for paying any withholding tax (and related interest and penalties) required by the U.S. taxing authorities in connection with payment made by with IPNIC DBA xxxxxxxxxx.com to the affiliate.

I have read the above Foreign Certification and herby certify that “I” “the affiliate” do/does not have U.S. activities, as defined above, and agree to the other terms of this certification.
