the check you issued last month has not arrived yet till now.

as I am going to move to a new house, would you please kindly send out the check again, to the following address?

更地道和直接一点的翻译如下:

Dear Sir/Madam:

We haven't received your last payment.

We will move to a new premises therefore please re-send the last and future payment to the following address.

We are looking forward to hearing from you.

supercrasy 翻译的时候还是最好说明是 支票没有收到。 另外please re-send the last and future payment ，这个地方也应注意一下动宾搭配。 last payment 可以说resend, future payment 说resend 就不合适了。
